I started out building the form out of plywood in my garage. You can see that it is about eight feet tall and four feet wide.

I next added chicken wire and pieces of rebar for strength. Also added PVC pipe on the bottom to protect the wood edges.

When the basic form was complete, I moved it out into the pit where I started placing the concrete in the shape I wanted the water to run.

Part of the reason for building this is to attract birds so I wanted both sound, as well as places for the birds to bathe. Here I am adding scrap pieces of PVC pipe to help fill in areas that would be lightweight and not take up as much concrete. In some cases the PVC also acts as conduit for running water through the concrete to come out a different place.

One of the first test runs for the water that will be shooting up from the outside of water pit onto the fountain.

You can see on the back of the fountain, I have built in a door to the inside. There are two pumps located inside. One is for running water into the PVC pipes that shoot water up on the outside. The other pump is to push water up though a pipe on the inside to the top of the structure to allow the water to flow around on the outside. All the flow is diverted away from the back so no water will be wasted on the side that isnn't seeen and to keep the door dry.

Here is the near final product. After I had all the water flowing the way I wanted, I let it all dry and painted it with a waterproof light blue color which you can see in the top image. The entire structure is elevated on bricks to keep the plywood out of the water. The bottom of the pit is filled with river rocks.
